Okay so if somebody, is being spiritually attacked and blurts out something blasphemous by accident, is that a sin.
 
This is judged by the One who reads hearts.

He said to them, “Are even you still without understanding? Do you not realize that everything that enters the mouth passes into the stomach and is expelled into the latrine? But the things that come out of the mouth come from the heart, and they defile. For from the heart come evil thoughts, murder, adultery, unchastity, theft, false witness, blasphemy. These are what defile a person, but to eat with unwashed hands does not defile.”
Matt. 15:16-20

The Just Judge knows the origin of the things we blurt out. That is what we need to look for when we examine our consciences, too. There isn’t an external yardstick for it. More to the point, there is no need to classify something as venial or mortal in order to confess it. It is hard to fight sin if we don’t examine where our sin comes from and repent from those things, though.

Exactly. Heaven knows if someone has OCD or Tourette’s or any of a number of other things that someone looking at externals only would have no way of knowing. An athlete in a wheel chair has a different kind of training than an athlete with use of their legs, but they are no less athletes! So it is with being trained up spiritually. These are things to talk about with a confessor who has knowledge of your trials and conditions. Everyone wants to continually advance in holiness, of course, but not everyone is on the same terrain.

Today I ran across this quote attributed to St. John of the Cross (but without attribution of where he said it, so I’m not so sure!), and I think it’s great:
“He who as himself as guide has an idiot as a disciple.”

Be willing to let go of every trivial or petty clinging you are still encumbered with, but never doubt the mercy of God. Even stumbling pleases, provided it is intended to go in the direction of the arms of God. And remember that we are all parts of a body because we are all meant to be one as the Trinity is One by being in community. It is never just “me and Jesus.” We are meant to rely on each other as part of the economy of grace.

I’ve been tormented by blasphemous thoughts, it been happening since I’ve been 15
 
You need to take up issues such as this with your confessor - whether he is your pastor or another priest. The internet is not the place to ask about personal moral issues.

And if there is a physical component, you need to speak with a proper medical authority; no one here is capable of diagnosing such issues.

Regardless of where the temptation comes from, it is no sin to be tempted. Our Lord was tempted to worship the devil. Few of us respond to temptation with Our Lord’s decisiveness.
If something flies out of your mouth that you regret, do what you’d do if you did the same to anyone else. Apologize. Say you didn’t mean it. Say that you love him. The difference with God is that God will not doubt what you say. He will never tire of forgiving you. He will always prize that day when you win victory over whatever torments you. Be sure of that: Christ died to win that victory for you. God won’t ever give up on you.
