I interpret the lukewarm as people who claim to be Catholic (or Christian) but who only accept God on their terms. In other words, cafeteria Catholics. This can also apply to some Protestants who, having only the Bible, reject the parts of God’s word that they don’t like.
“If I say to the wicked, `You shall surely die,’ and you give him no warning, nor speak to warn the wicked from his wicked way, in order to save his life, that wicked man shall die in his iniquity; but his blood I will require at your hand.” - Ezekiel 3:18
“So, because you are lukewarm, and neither cold nor hot, I will spew you out of my mouth.” - Revelation 3:16
