Yes, they have existed forever. Heaven is being in the presence of God and Hell is being outside this presence. Heaven is when we are in communion with God, Hell is when we reject and ignore God. So, technically, as long as there is God, you will have the choice to love or reject him.

Heaven is when we are in communion with God, Hell is when we reject and ignore God. So, technically, as long as there is God, you will have the choice to love or reject him.
However, I believe hell came later. It makes sense to think that hell came into existence therefore by the fallen angels as they chose to reject God.

Heaven and Hell are in eternity, therefore they have always existed.
They are not just “states of soul” given that a) angelic beings, who are not human souls, inhabit both, and (b), spiritualized human bodies (pneumatikon soma) also inhabit both. They are therefore “places” in a way outside our experience.
They are not in our universe, because they are eternal, and our universe is bound by entropy. They are another frame of being, which is more than a state of soul. The eternal worlds have been described as higher dimensions.

How did hell exist forever when satan was once an angel along with the other angels that followed him in the fall?Heaven and Hell are in eternity, therefore they have always existed.

Who then was in hell? What purpose did this hell serve if nobody was in it and God is all loving and merciful? There was no need for any punishment, so why would it have existed?

Hell is a state of being of an individualized consciousness (human, angel, e.g.). Before God made humans or angels, there was no hell.
Likewise for heaven.
