How and when did confession in the confessional develop?

How and when did the sacrament of confession develop into the present means of confession in the confessional box? How difficult was it to confess your sins in the early church, and some of the penances? Who were some of the saints in the developing of the present day formate? Was the seal of confession from the beginning?
