How do I change my name on my sacramental record?

I was baptized at one-month-old under my given name. When I was 4-years-old, I was adopted and my last name changed on every document I had (birth certificate, Social Security). It was a closed adoption and no one can trace me back to my biological father who is in prison. When I got married in the Church and later had my children baptized, my biological last name was on the certificates, much to my dissatisfaction. The parish secretary told me these were the rules and would not change it. It makes no sense to me because I have never gone by this name and no one knows me by this name.

Call your local diocese and ask if there is a way that the bishop’s office can make the change. You might also consult with the St. Joseph Foundation.
