How do I convert to the Catholic faith?

I have been doing soul searching on this…my boyfriend is a Catholic and I am a Pentecostal Christian…having talked a lot with him about his Denomination and with our plans to be a family with one faith i have been wanting to know how to convert to Catholic…what does it entail…and having been previously married can I be baptized catholic?
