How do I get back to the church?

I’m a baptized Catholic, I married a baptized Catholic. We have two wonderful girls that have not been baptized. We used to go to our local church frequently when we lived in Houston (over 5 years ago); however, we haven’t gone since moving to the Chicago area in 2000. I blame myself.

My question has several components. What do I need to do to get my girls baptized (ages 6 and 9) and all of us confirmed? I know what Parish I’m in., do I just set an appointment with the Priest?

Also, I would like to make a confession, it would be my first. I have sinned and have received forgiveness from those I hurt. However, I need to make it right by God.
