If you were fully initiated into the Church (baptism, First Communion, confirmation) at the time you left, all that is necessary is confession. If you lack one or more of the initiation sacraments, then you would go to confession and meet with your pastor to begin the process to receive the sacraments you yet lack. Welcome home!
