Is birth control ever allowed?

I am a mom of five children, one of whom is autistic. It is a medical recommendation that I do not get pregnant since I have previously had two cases of blood clots in my legs postpartum and that I have had a hysteroscopy and polypectomy recently. What do I do? My husband insists he will not have a vasectomy and this clearly rests on my conscience as a Catholic . I am 42 and have a long way to go until menopause. I have considered abstinence for the remainder of my child-bearing years but that is a very long time and not very practical. Please help me understand what the Church teaches in these instances.
