Small correction: if you have already received Holy Communion twice that day you should not receive.You may receive at any Mass if you are in a state of grace (no unconfessed mortal sins), but you are not required to receive.
I would highly recommend you get to confession.I haven’t taken communion in close to a year because I’m not in a state to receive.
I think it would be something you should bring up when you get to confession, but I would not consider it as serious as receiving unworthily.Is there a penalty for going past that without receiving?

It does not. The law says receive communion. There’s only one thing that constitutes receiving communion, and that’s actually receiving communion.I’ve no idea if this covers you on the once a year rule though.
Frequent Communion has always been recommended by the Church. In the early Church, one was expected to receive if one was present for the sacrifice. By the early Middle Ages, the faithful became less and less comfortable with frequent reception of Communion, but infrequent reception was never considered ideal by the Church. It became such a problem that the church had to mandate a minimum of once a year.Prior to the pontificate of St Pius X, frequent communion was largely unheard of.
