Is it ethical for me to file questionable medical records?

I work in a medical records dept. I sometimes may have to file consents for sterilizations into patient charts so that they are available for the surgeon when the patient is in the OR. Is this wrong?
What about Living Wills, where the pt has stated they do not want artifical feeding and rehydration? Is it wrong for me to put them into patient charts?
 
Dear Mairegal,

The record will be filed whether you are the one who files them or not. You are not the one who is determining whether a person decides to be sterilized or not. This holds for the second situation as well. You are not culpable if you file the records.
