Is it true that "Once a Catholic, always a Catholic?"

I was told that when someone wants to cease to be Catholic, they have to petition the Vatican if want to officially leave the Church. My response is if they want to cease to be Catholic they likely never were Catholic! What’s the real answer? Thanks, DB
