You don’t. It is in God’s hands. Except for the saints, which the church declares are in heaven.

Except for the beatified or canonized saints, you cannot be assured that a soul has reached heaven. One should never stop praying for the repose of particular souls, such as those of parents, other relatives, or friends who have died.

As others wrote, you don’t know except for the saints the Church recognizes. However, your prayers are still effective because purgatory, like heaven, exists outside of time. Even if your loved ones are in heaven, your prayers can somehow aid them back when they were in purgatory. This also applies to people we might think are in hell. Even if they died 20 years ago, our prayers can aid them just as they are dying and meeting Jesus as if their death just happened.

How do you know when a soul reaches heaven
You generally don’t know for sure (canonized saints excepted).
If it is very important that you know, you can try praying to God with your question.
He may answer, then again He may not.
