Latin prayer translation needed

Does any know the Latin translation of the Eucharistic prayer “O Sacrament Most Holy, O Sacrament Divine, All praise and all thanksgiving be every moment Thine”?

I can’t find it anywhere. I’ve tried a handful of online translators, and each gave a different result.

The first line would be “O sacramentum sanctissimum, O sacramentum divinum,” but my Latin chops aren’t nearly good enough to do the second line. I did google the first line (Latin) as a text string, but got nuthin. Sorry 😦

I believe it was originally in Italian, not Latin; this is how it was given in the Raccolta of 1834:

This 1877 source indicates that it was this “Italian rhyme” that was indulgenced. Then in a 1913 decretal in the AAS we find the same “authentic” Italian prayer reconfirmed, with the same indulgence granted for praying with the “prece quae sic sonat: Laudetur et adoretur in aeternum sanctissimum Sacramentum”; so there is your Latin translation, and it is in that form that we find it, among other pious invocations, in the present Enchiridion Indulgentiarum.
