Now, 23 years later, the 46-year-old says he felt trapped as he was conscious the entire time and attempted to communicate.

I can imagine it. I know that people in medical training are always told NEVER to speak negatively of a patient’s condition in his presence, even if he’s comatose, because it is not uncommon for the patient to hear and understand every word notwithstanding that the patient seems to be totally “out of it”.
 
This certainly has some troubling implications, quite apart from the horror of the idea of conscious experience in a body completely unable to move or communicate. One wonders how many times similar circumstances have occurred, where life-support was withdrawn, nobody realising that the person was still aware of their surroundings. Clearly the medical definition and indications for what’s called a ‘persistent vegetative state’ must adapt to become more stringent. It appears there is a need for more exacting tests to determine the precise condition of a comatose patient. As one doctor remarked in the article, technology finally caught up with this man - as, presumably, did assumptions about neural activity. At least he now has the means with which to communicate to others.
