Some men have noticed that while doing abdominal exercises, blood flows to the private area and may cause an erection and/or ejacualtion. If a husband wanted to keep his abdominals in shape as something to offer his wife and he knew that sometimes it caused ejaculation but was not done with the intent of actually ejaculating, would this be a grave sin?

I guess no more than a wet dream would. Depending I suppose on how frequent the occurence and whether, for example, alternative exercises might be found that would avoid the problem.

It’s really an issue to be considered in consultation with one’s confessor.

It would be approximately once a month (not planned). I already tried alternative exercises but they’re either not as effective or they end up having the same effect over the long run.

Same as if you’d woken up in the middle of the night and you’d had a wet dream…lack of intent to ejaculate alleviates responsibility here.

Intent is everything…if you can be perfectly honest with yourself and say that you’re simply doing the exercises for the purposes of taking care of the body God gave you, you’re fine and don’t worry about it.
 
Not to get too graphic here, but I would imagine it’s an issue of friction and clothing. That being said, you might consider looser fitting workout attire. That’s pure guessing on my part as I’ve never experienced this problem while working my abs. Perhaps I’m not doing something right.🙂

It’s the abwheel. It works out your abs so hard that it happens (at least to me).
 
A couple of things to consider:
  1. Use another form of ab exercise. Fellows were able to achieve six-pack abs long before the abwheel came into being.
  2. While looking good for your spouse is noble, at what point does it cross the line and become vanity or a form of pride?
That’s a little off point from your original question, but it might be something worth taking a peek at as you examine your conscience.

The church certainly does. From the Catechism:

"[2352] By masturbation is to be understood the deliberate stimulation of the genital organs in order to derive sexual pleasure. "Both the Magisterium of the Church, in the course of a constant tradition, and the moral sense of the faithful have been in no doubt and have firmly maintained that masturbation is an intrinsically and gravely disordered action."138 “The deliberate use of the sexual faculty, for whatever reason, outside of marriage is essentially contrary to its purpose.” For here sexual pleasure is sought outside of "the sexual relationship which is demanded by the moral order and in which the total meaning of mutual self-giving and human procreation in the context of true love is achieved."139

To form an equitable judgment about the subjects’ moral responsibility and to guide pastoral action, one must take into account the affective immaturity, force of acquired habit, conditions of anxiety or other psychological or social factors that lessen, if not even reduce to a minimum, moral culpability."

The basic reasoning is that masturbation is a sex act that doesn’t adhere to the God-given purposes ordained for sex. These are 1) unitive (deepening of the bond between spouses within the marital relationship) and 2) procreative (in accord with God’s command to be fruitful and multiply).

Anything that frustrates either or both of these purposes - be it masturbation, contraception, adultery or premarital sex - is a grave/mortal sin.

Whether there are lustful thoughts accompanying any of these activities is beside the point, they are, as I said, contrary to the purposes for which the Church teaches us that sex has been designed.

Don’t let him kid you - masturbation always involves some impure thought or lust.

Accoding to the Catechism ir has been the “constant teaching” since the time of Christ that sexual activity is reserved for the marital act. The question is why does Dr. Dobson think he’s in a position to define his own moral teaching because it’s easier to following than 2000 years of moral understanding.
