What is the understanding behind “grave matter”? How do you determine whether something is grave? It seems a little ambiguous to me.

it depends on the intention. if there is an intention to offend God or offend others or even yourself (self mutilation, suicide, etc.) then it is grave matter. you may kill someone but never intend to, maybe as a result of self defense or an accident, then its not grave matter. but you can do something really trivial but your intention is to offend God, then it is a grave matter.

The Catechism of the Catholic Church defines grave matter as:
Code:
**1858**. Grave matter is specified by the Ten Commandments, corresponding to the answer of Jesus to the rich young man: "Do not kill, Do not commit adultery, Do not steal, Do not bear false witness, Do not defraud, Honor your father and your mother." The gravity of sins is more or less great: murder is graver than theft. One must also take into account who is wronged: violence against parents is in itself graver than violence against a stranger.

You need to discuss it with your priest. Remember that it is not so much what we may consider ‘grave’, but what the church considers grave moral offenses.

For instance, you might not think that viewing internet porn or an of impurity are that ‘grave’ … surely they are not on par with murder. But both are mortal sins.

Again, when in doubt take all your sins to Confession and your confessor will help you with spiritual guidance.

“Grave Matter: The act itself is intrinsically evil and immoral. For example, murder, rape, incest, perjury, adultery, and so on are grave matter.” - Catholicism for Dummies Pg 207
But it would be something that would hurt - the victim, the doer, or anyone else. It would cause hurt to anyone and possibly anybody.

You ask your confessor.
My confessor says that watching pornography is a mortal sin and masturbation. Both of these offend God.

The number one mortal sin is delibrately missing Mass on Sunday. I got chewed out in whispers by a 90 year old priest for this one years ago. It made an impact on me. It truly was the Holy Spirit speaking to me in the confessional.
