Mortal Sin

I know there are three elements for a sin to be considered mortal but I forget if they ALL have to be present or does just one of the elements have to be for it to be considered mortal?
I’m having a brain freeze right now and can’t think of the answer.

All must be present. If one is missing, it is venial. Venial sins still need to be forgiven so we can be more Holy. Also, the cumulation of venial sins can so damage our relationship w/ God that we can easily fall victim to mortal sins.
