Is a woman who was sexually assaulted as a child – and thus lost her physical virginity – permitted to become a consecrated virgin?

The requirements do not appear to forbid it. In any case, having been sexually assaulted does not in any way alter one’s status as a virgin. St. Joan of Arc is widely believed to have been sexually assaulted while in prison (source; see the section sub-titled Execution) and she is honored by the Church under the title Virgin.
