Offering your penance

For my penance, I was told to say it also for the sick and dying. How do I do that? Do say “I also say my penance for the sick and dying” before I start my penance?
 
Before I start a penance or a Rosary or when I have trials, I say, "Eternal Father, I offer this ______, and I ask You to join it with the sufferings of Jesus in His Passion and use it according to Thy Will, for ‘the sick and the dying’ or ‘state intention’.

That’s odd. I can see saying prayers for the Holy Father’s intention, but why for State intention? Isn’t that a violation of the Establishment clause?

I believe what is meant by state intention is that you state (announce) the intention you want to pray for. 🙂
