Personalizing stationery for a priest

A close family member is a priest and we will be attending his installation as pastor of a new church (new to him, not brand new!) We would like to order him personalized stationery with his new church’s name and address. Nothing too formal, something lighter for everyday use. I would like to make sure I follow proper protocol and would like to know how to personalize. Would it be Pastor John Doe, St. Mary’s Church, 123 Main St, etc?

Thank you for any assistance anyone can offer.
 
The Rev. John Smith (initials if he is in a religious order)
St. Mary Catholic Church

in the address line inside a letter
Rev. John Smith

Dear Father Smith:

If he is a monsignior (an honorary title)
The Rev. Msgr. John Smith
address

Rev. Msgr. John Smith

Dear Monsignior Smith

If he has indicated he prefers it, still address him as Father

In addressing him in conversation he is Father Smith or Monsignior Smith (unless he tells you to call him Father John).

He will probably order his own business stationery, in which he might add the title Pastor after his name, but probably not for personal stationery, as your gracious gift will be. Catholic priests are not generally at least in this country addressed as Pastor
 
If you have access to an image of his coat of arms (if he has one) you can have that embossed on the stationery. (Just a thought)
