How should I talk to someone who truly believes that the wrong gender was assigned at birth and desperately wants to undergo “sex reassignment” surgery?
 
Politely, and with humility.

I think both are essential to anything you might say regarding a topic which is so deeply personal. How well do you know this person? How deep is your friendship?

I would add that you can’t likely know what is going on in any biological or psychological sense, so you should avoid any impression of judgement. Listen and sympathise, offer prayers for the person to find peace and trust in the Lord that the right thing should be done.
