Is it appropriate to ask a priest to receive the Annointing of the Sick sacrament due to eye disease and/or increasing visual impairment? Or does the illness need to be life threatening/grave?

My understanding is that the condition needs to be serious, but it need not be life-threatening. Even partial deafness or blindness: yes, assuming the condition is not reasonably expected to be transitory. Runny nose or skinned knee: no. Ask your pastor about it.

of course, any disease or condition that materially affects health and ability to function, does not have to be life threatening.
