What are some things to tell if a relic is real outside a COA?

You can’t know. Even real relics are not always able to be authenticated they are often just taken on faith to be what they claim if they are past a certain age. I like to file them in a mental box marked ‘just toooo Catholic for me’

If you obtained it yourself for your personal use, such as by touching your rosary to a saint’s grave to make a third class relic, then you know it’s authentic.

The official cause websites of servants of God and venerables, and sometimes their shrines, provide relics (second or third class) you can rely upon to be authentic.

There a many things to look for. There should be a wax seal on the back under the cover with threads running through it. Threads should be intact and the imprint on the seal should be from the issuing authority. The age, design, and wear of the metal case should match the era the relic might have been issued, if you could infer that. Thr label inside should match too.
