Do you need to intend to separately ask for forgiveness when you say the prayer or do you need to only say the prayer? (after all the “Our Father” explicitly asks to forgive sins anyways so just saying it w/the intention of saying the prayer should be enough).

I think what you are asking is do you have to say a prayer for each and every venial sin of which you are aware or can you just lump them all together and take care of them with a single prayer? If that’s the case I vote for the “one prayer does it all” option. I base this on the “penitential rite” at the beginning of Mass. We say one prayer and all our venial sins are forgiven.

At the start of Mass I take a few minutes to go over my week and call to mind all my venial sins. That way when we get to the penitential rite I can just do a mental reference to “all that stuff”.

Also - the Church recommends that as part of our nightly prayers we include an examination of conscience for the day’s sins - along with an act of contrition for them. That helps to strengthen our intent to not sin again.
