First question:
Can your rosary prayers still be heard when your mind strays from the mysteries? If i finish a decade and realize i was meditating on the mystery for only 2 of 3 beads, should i redo the decade or just keep going?
Plus, lately i’ve been saying the rosary in latin, and reading the prayers off paper, so it is hard for me to concentrate on a mystery.

And second question:
How are you supposed to phrase your intentions at the beginning of the rosary? Are you addressing Mary or God? Do you say: “i am praying this rosary to ask you ______?” or what?

Just do the best you can. God and Mary will understand. If we were to go back and “redo” every time our mind wanders, we might never finish! Just gently bring yourself back and continue.

The way you are stating your intention sounds fine to me. Sometimes I like to relate the intention I am praying for to the mystery I am meditating on (e.g., the sick person I am praying for is like Jesus suffering His agony in the garden, the Joyful mysteries help me pray for families, children, and reverence for life, etc.). I just bring that person or situation into my thoughts on the mystery.
 
Here is a prayer you can use if you want to, to begin your Rosary. I use it every time. It is from the Fatima, Portugal apparitions.

Queen of the Most Holy Rosary, You have deigned to come to Fatima to reveal to the three shepherd children the treasures of grace hidden in the Rosary. Inspire my heart with a sincere love of this devotion, in order that by meditating upon the mysteries of our redemption which are recalled in it, I may be enriched with its fruits, obtain peace for the world, the conversion of sinners and of Russia, and the favor which I ask of you in this Rosary (here make your petition). I ask this for the greater glory of God, for your own honor, and for the good of all souls, especially my own. Amen.
