Sin leads to death, both natural and spiritual. Many die as a direct result of their own sins, and even though they may not have intended their own deaths, they knew or should have known the risks of their actions and chose to do them anyway. The result is, if you will, depraved heart suicide. It is no surprise that such people often die in their sins, because death takes many of them while they are sinning.
 
…And to add to that, he makes the point, that for some people, the final agony will be so consuming that there won’t be time for peaceful reflection or repentance.
