Searching for a particular hymn

During the Season of Easter at the church I was going to near my college sang a hymn during communion called “Choose Life”.

As anyone ever heard of it or know of the composer who wrote it? I’m trying to find the sheet music or a recording of it.

No, that’s not the one

The one I know of goes something to the effect of

“Choose life, keep faith. Remember the promise, out of the darkness and into the light. Choose life, keep faith. Remember the promise. You are my people and I am your God.”
