“Very often” seems strong. I’ve been going to NO my whole life and I can count on one hand the number of times I’ve seen it. Granted, one of those times was me. 😑
Yeah, in about 30 years of Masses I saw it happen exactly twice when the person distributing Communion (in one case a priest) fumbled.
Same here. About twice. Once was someone receiving on their tongue who sort of scraped It off with their teeth. Awful.

I am 17 an have been going to NO all my life, and have only noticed it be dropped twice, once the elderly priest put it too close to the tips of my fingers and it tumbled off, the second time we noticed one to be on the ground around the time of the Lamb of God/Lord I am not worthy… part, no idea how it got there (or when) possibly when the priest put a ciborium to the side (where I am, Cup and Host 1 are usually given the chalice and ciborium immediately after those two have received the cup so that the choir can receive communion while the EMHCs and altar servers do, and the EMHCs recieve the vessels, and our altar is shaped kind of like the outline of 1/2 a hexagon so there are two “wings” closer to the EMCHs than the main part used during the mass).

Look to the priest - some people will ‘fiddle’ so much the host will just break up (and some hosts are poorly made sorry to say). A competent MC/server should be able to either cover the host or remove it and then dissolve it in water. The priest shouldn’t try to pick it up off the floor as he may have more crumbs/particles on his fingers which will be left on the floor. There’s a reason for priests keeping their thumbs and index fingers held together.
