In the Catholic church there are various ministries that involve adults with minor children.
This is true of adult lay men serving with minor boys.
What is the official name of the consent form that adult servers have to sign before they can serve with minor boys?

In our state that would be a CORI form/investigation. And it’s all minors, not just boys. And it’s all contact, not just altar serving.

I think the title varies by Diocese.
 
Many dioceses use the “Protecting God’s Children” program from VIRTUS. Many will also use programs called “Safe Environment XXXXXX”. It completely depends on the diocese.
 
In Australia you need a Blue card to deal with minors ,even school cleaners need a blue card
 
So I guess the correct answer would be: it depends on your location and jurisdiction. Talk to your pastor, and he will guide you to where you need to go.
 
The only thing we have to sign is a consent to a court check and a police check that includes a “vulnerable sector” check. I was required to have a regular police check when I was the parish secretary and handled money. I also did the vulnerable sector check because I sometimes helped with sacramental preparation.

Our diocese requires no special training to work with children.
