Suggested prayers for nursing home ministry

In a few weeks I will be starting to function as an extraordinary minister of Holy Communion. I will be bringing the Eucharist to patients in a nursing home. Is there a guideline for what prayers should be send prior to giving Communion to each patient. I think that most of the other ministers just say the Our Father, but I haven’t been given any specifics. Any ideas would be appreciated.

You should recieve a small book from your pastor with pray options for communion of the sick.
