I know that it is common knowledge that St. Polycarp was the bishop of Smyrna. Nicolas of Lyra claims that Polycarp is believed to have been bisop of Pergamum also. Here is a quote from Lyra, *
And to the angel of Pergamus
That is, to St. Polycarp, as the doctors commonly say. Nicolas of Lyra Commentary on Revelation] 1270-1349AD *

I have not been able to locate any other witness to confirm this. Anybody know?

Your quote does not mention Nicolas claims to have been Bishop of Pergamum. Is that somewhere else in his writings? He refers to him as the Angel of Pergamum, not the Bishop. Maybe he uses this expression because Pergamum is where St Polycarp is buried?
