According to the RSV-CE, the curse is to make a woman’s body swell and her thigh fall off! Ouch.

Any ideas what this means?

it means she needs to lay off the salty foods and not wear her jeans so tight

it seems to be in the context of not only the punishment for a woman taken in adultery, but an ordeal to determine her guilt.

The 1985 New Jerusalem Bible, noted for conveying a sense-for-sense as opposed to a word-for-word translation of the original languages into English, renders it thus:

5.22 May this water of cursing entering your bowels, make your belly swell and your sexual organs shrivel!

the word ‘thigh’ is probably a euphemistic referal to the sexual organs.
