what are the reqùirements needed to marry in church if we are already civil wed for 15 yrs.?

Hi my husband and i is already married in civil rites for 15 years now and we intend to have church wedding this coming May .I just want to know the requirements since our parish church says we only need our marriage certificate and an appearance in the parish before the wedding but our friend says otjerwise .please.enlighten me since our time is very limited to complete.other requirements.thanks.
 
The usual requirements are:
  • Proof of civil marriage
  • Testimony from witnesses that you are both free to marry (i.e. no previous marriages)
  • Proof of baptism
  • Agreement to the Church’s understanding of marriage
