What does the Church say about Living Wills?

I heard recently that there is a potential problem for Catholics with the standard “Living Will,” and that there is an alternative form for us. It clarifies things like whether or not to intubate a comatose patient, when a DNR is permissible, etc. What is the name of the form and where are they available?
