What is meant by approaching the Eucharist?

When Catholic books, scholars, etc. say that someone may not approach the Eucharist, does that mean they cannot receive, or that they cannot go towards the Eucharist, even with their arms crossed?

However, I really don’t know why the euphemism is used. Perhaps it’s an idiom in Latin or Greek, or perhaps long ago not all who approached were permitted to receive.
