I have always understood that the correct form, when writing to or about a Roman Catholic cardinal, is “His Eminence, John Cardinal Doe.” Yet, in many cases, I see “Cardinal John Doe.” Which is correct or preferred?

Either form is appropriate. But if you are speaking to a cardinal (as distinguished from speaking about him), you would call him “Your Eminence” (not “His Eminence”). You can also call him “Cardinal.”
