What is the meaning behind "Lord, hear our prayer."?

For example;
“for the homeless that they might be kept warm this winter.”

“Lord, hear our prayer.”

Since its not an actual prayer when Catholics say the phrase
Lord hear our prayer (as in like please heal so and so’s leg.") is the meaning something like God you know what I’d pray for already so amen."

As someone who was not raised Catholic…

For the homeless…

Lord hear our prayer.

What prayer? What does that mean?

Wheres the …Father please keep the homeless warm and provide them with plenty of food…

It goes directly to Lord hear our prayer.

These kinds of statements are usually made during the Mass during the Prayers of the Faithful. They are communal prayers.

It just means “please hear us and grant our request”.

These are response’s to the many petitions the Catholic Church has. It unites the faithful to the body of Christ. We are “all in” so to speak. The Devine Office, the official liturgy of the Church has many of these petitions daily. ALL Catholic Priests are REQUIRED by oath of their office under the penalty of mortal sin to offer these petitions daily for the1.2 billion Catholics on earth in the name of our church… Pretty awesome. It’s a prayer.
Sometimes the communal prayers use a different phrase, like “Lord, graciously hear us” or something else.
The intent in every case is that the whole congregation answering is praying together for whatever the stated intention is.
