What is this medal, and what does the latin phrase on it mean?

Randomly found this in my house just now. The phrase looks very similar to “in hoc signo vinces”, but apparently it’s not?

Yes, it is “In hoc signo vinces,” except that the G in “signo” has been miscopied as an upside-down D. It’s possibly a copy of a Portuguese or Brazilian coin. That particular cross is the emblem of the Order of Christ, instituted by the Portuguese monarchy as a successor organization to the Knights Templar, and frequently seen as a symbol of the Portuguese state.

The original design did not have the date on it.

BUT in 1738 a gold coin with this exact design was coined by Portugal then a Kingdom weighing 0.964 grams, this would have been the reverse side of the coin in the front the word IOAN and a royal crown over the word would have been stamped. Its face value was of 400 Reis.
They are available from numismatic dealers aprox value of US $300
