What must a doctor provide?

If they choose not to provide a service for moral reasons, they are ethically required to provide an effective referral to another practitioner.
What comes to my mind is something like this: “I won’t kill you, but I can refer you to somone who will.” or “I won’t kill your unborn child but I will refer you to someone who will.”

That does become the difficulty for Catholic physicians - what constitutes an “effective” referral? Is referring someone to an information line where they can obtain the name of a practitioner “effective”? Or do you, yourself, have to refer to a doctor for an abortion or for assisted death? The former, I believe, is generally considered morally acceptable while the latter, for obvious reasons, is not.
