When were the Sacraments still administered using the older forms?

I know that not just the Mass, but also the forms of the administration of the Sacraments, changed following Vatican II. When were the rituals for Baptism, Confirmation, Marriage, and Holy Orders changed?
 
These dates refer to the dates that Latin texts were issued. In addition, this is only the first edition: some were further revised in later years. Some of these dates also represent December dates, so at times, I’ve simply given the year following, in accordance with the date of publishing. These are also not the dates set aside for implementation of the rites. Generally a period of around 1-2 years from the year given was allowed for their adoption.

Baptism
---- of infants - 1969
— adults (i.e. the initiation rites) - 1972
Confirmation -1971
Penance - 1974
Anointing of the Sick - 1972
Ordination (bishops, priests, deacons) -1968
Marriage- 1969

Lastly, the rites did undergo some [rel.small in comparison] modifications prior to their complete revision (one extensive set of revisions is found in Inter Oecumenici)
