Where did "Lord, I'm not worthy to receive you . . ." come from?

I’ve been having discussions with a former Catholic, now Baptist, who says the Catholic church is not really the church Jesus started as in “it changed”. I had heard at one time the response “Lord, I’m not worthy to receive you but only say the word and I shall be healed” was started by an early church father but can’t recall who. Thought this might be valuable info to show how little the mass has changed.
 
Although recitation of this phrase during the Mass is not an ancient practice, the phrase itself is derived from the words of the centurion recorded by Matthew: "Lord, I am not worthy to have you come under my roof; but only say the word, and my servant will be healed” (Mt 8:8).
