Which Eucharistic Prayer, etc.?

I hate to put my ignorance on public display :), but can someone enlighten me as to how and by whom it is determined which Eucharistic Prayer is recited at any given Mass? Same question with regards to the Penitential Rite. Is there a rotation, or a formula, or is it randomness, or the personal preference of the bishop or celebrating priest, or what?

It is completely up to the priest, however suggestions are given in the GIRM for when it would be most appropriate to use a particular Eucharistic Prayer (i.e. Sundays, Feast Days, Sundays in Ordinary Time, Weekdays)
 
The celebrant has the choice when it comes to the penitential rite as well as the Eucharistic Prayer. As long as they are using one of the approved forms in the approved way, it really is up to their preference.
 
It’s the choice of the priest. Obviously if they are concelebrating, it helps if they agree in advance. There are special forms of Eucharistic Prayer I for particular occasions and it seems a shame not to use it.

One of our priests is of the opinion the Eucharistic Prayer II is not really suitable for Sundays and another who would use it at every Mass if we didn’t threaten him with nailing the Missal open at Eucharistic Prayer IV.

It’s the personal preference of the priest-celebrant for that particular Mass.

I, II, and III can be used at any Mass without restrictions.

Keep in mind though that when it comes to E.P IV, that one cannot be used on days which have a “proper preface” --which means a preface required for that particular day (as opposed to one for the season).

There are some very general patterns like using EP I on more solemn occasions, and using the shorter ones on weekdays. These are merely patterns, not requirements.

There are also some recommendations in the GIRM that the priest choose EP I on certain days–specifically those days where EP I has special prayers, or days of the saints mentioned in the canon. These are suggestions, not requirements.

Which form of the penitential rite is completely a decision of the celebrant–no restrictions here.
