Will one unconfessed mortal sin condemn us to Hell?

I have returned after many years to my Catholic faith. My question is : if I have committed a mortal sin while trying to live in the path of Jesus, am I condemned to hell if I die before having confessed this sin? I have confessed it before but struggle with trying to overcome it.
