You are thinking of surgical transitioning. Most people with gender dysphoria to not go through surgery and if they do it is the less invasive types, like mastectomy or face feminization.

I believe if there was greater acceptance of transgender folks fewer would desire surgery.

If someone says “My name is John”, and you insist on calling him “Beth” regardless of the fact that it upsets him greatly then you’re not “upholding truth” or “defending the truth”, you’re just being a bully.

Do they? Or are they dysphoric? They are not delusional about their anatomy, but it doesn’t fit with their brain.

No, they already feel like one. And not all do “cross dress” sometimes they dress androgynously; I know of a transwoman who still plays around with wearing makeup and very feminine clothing.

They might tell you that, but mostly they will live their lives and won’t pay you no never mind.
