Real-world cost: We are paying over $1200 a month for a high deductible HSA policy. Actually 2 policies because the first company who would accept me, with an upcharge of course, would not accept our younger son, so we had to get another policy for him alone.
Our deductible is $5,000/year. In effect, except for catastophes such as accidents and cancer, we have no insurance. We pay cash for medical appointments and medications.
