Guidelines for prayers of the faithful

Who is usually responsible for these in your parish and are there guidelines or rules for them?
 
Also in the Roman Missal there is Appendix V “EXAMPLES OF FORMULARIES FOR THE UNIVERSAL PRAYER”. There are 11 examples there.

Another document about this is “The Universal Prayer or Prayer of the Faithful” 2nd edition, 17 April 1966. It begins with “The Constitution on the Liturgy has taught and decreed” and is at http://www.pford.stjohnsem.edu/ford/courses/sacramental-theology/docs/Universal Prayer.pdf . It is also in the book Documents on the Liturgy 1963-1979, ISBN 0814612814. There it is Document 239, on page 594.

In my parish we usually use a few that appear in the missalette we use. The last one is always for our own personal intentions with a moment of silence to allow us to silently offer those requests to God.

When I was the secretary it was my responsibility to prepare them, now a volunteer does that. Since our parish includes a military base with no Catholic chaplain making us their Catholic Parish, I always added one for the military, chosen from a selection provided by the Military Ordinariate.
