Is an annulment always necessary after a divorce in order to remarry if both spouses are living? If not, what are the circumstances involved?

Yes and no.

Before you are able to marry in the church you have to be declared free to marry. Depending on the situation of the prior marriage it may or may not require a full tribunal process for a decree of nullity. Some marriages get the “short form” because of a canonical defect like a Catholic who previously married at the JP for example.

Your question is not quite clear. Do you mean to remarrry each other or someone else?

Someone else. If you divorce and want to remarry the same person and have never had an annulment from that person then there is no problem - the Church considers you still married. All one would need to do then is have a civil ceremony in order to fulfill the laws of the state.

My understanding is that if a former spouse is still living, the Church must make a determination as to the validity of the prior marriage, before any subsequent marriage can be undertaken.
 
Technically speaking, the Pauline and Petrine privileges do not declare a prior marriage null, but they instead dissolve it. But you still need to go through the Church tribunal for these as well, so this distinction isn’t important as a practial matter.
