I am stumped. What to do if the husband is consistently, or a significant number of times, not cooperating to help the wife?

I don’t think what you are asking is permitted. It is generally allowed for the wife to stimulater herself to climax if this is not acheived during the marital act and the husband is not willing to assist her. I have even read where it is allowed during the act to help herself along so that they may climax together.
