Ballots can be tossed out when a person votes in person too. There are a few examples of ballot boxes going missing and then being found months or a year or so later.

Human error or deliberate? Who would know. The answer lies in better procedures with votes. Remember that entire second ballot in one state when the first was in error due to the procedure where voters clicked a ballot
